Certified Nursing Attendant

The CNA will deliver person centered care based on the needs and interests of the residents while preserving their dignity and quality of life. CNA provides assistance and support for the activities of daily living; including but are not limited to dining, housekeeping, and life enrichment, which are a part of the residents daily choices. CNAs work under the supervision of the Resident Care Director/Director of Nursing/Healthcare Administrator.

Cook

Cook/Prep Cook oversees various phases of food preparation, production and service. This position assists with menu planning, maintenance of kitchen equipment, sanitation programs, and overall kitchen staff performance under the supervision of the Executive Chef.

Director of Nursing

Director of Nursing establishes and implements standards of resident care throughout the healthcare system on a 24-hour basis. This position oversees all nursing aspects, directing and evaluating all nursing personnel providing person centered care within the skilled nursing and assisted living environment. The DON will deliver person centered care based on the needs and interests of the residents while preserving their dignity and quality of life. The DON oversees and provides support for the activities of daily living; including but are not limited to dining, housekeeping, and life enrichment, which are a part of the residents daily choices. The DON is part of the care plan team and oversees implementation and evaluation of resident assessments and individualized care plans using the web-based program Keane. This position oversees the documentation for residents in their neighborhoods, assigns residents to nursing assistants as needed and assists in the medication management and overall healthcare programs for the residents in a neighborhood. The DON operates healthcare services to be compliant with federal, state and local regulations. The DON coordinates and works with community agencies, physicians, families, residents, consultants, ancillary service providers and other departments within the community under the guidance of the Healthcare Administrator/Executive Director.

Housekeeper

Housekeeper cleans and sanitizes common areas, homes, apartments, suites, and healthcare units on the campus. Housekeeper will follow a regular work schedule to accomplish duties under the supervision of the Housekeeping Supervisor.

Licensed Practical Nurse

The LPN will deliver person centered care based on the needs and interests of the residents while preserving their dignity and quality of life. LPN provides assistance and support for the activities of daily living; including but are not limited to dining, housekeeping, and life enrichment, which are a part of the residents&rsquo: daily choices. LPN assists the care plan team in the preparation, implementation and evaluation of resident assessments and individualized care plans using the web-based program Vigilan. This position documents for residents in their neighborhoods, assigns residents to nursing assistants as needed and assists in the medication management and overall healthcare programs for the residents on a neighborhood under the supervision of the Director of Nursing/Resident Care Director/Healthcare Administrator.

Life Enrichment Leader / Activities Leader

The Life Enrichment Leader is part of the Member Services Department and collaborates with the Member Services Department to deliver outstanding customer service and develop an enriching community program for the members that integrates and meets member interests and needs into their daily routines. In order to meet the interests and needs of the members, the Life Enrichment Leader shall assist in interviewing healthcare members to learn about their preferences, lifelong interests, favorite things, and daily routines. Under the leadership of the Director of Member Services, the Life Enrichment Leader shall assist in the development and integration of social events, outings, programs, and activities, with a multitude of choices, for the members of the campus based on what they learn about members. The social/activities program shall be multi-faceted and reflect groups&rsquo: and individuals’ interests and needs and provide programming that focuses on the six dimensions of wellness: emotional, intellectual, physical, social, spiritual, and vocational. The program shall be a well tailored program that includes, but is not limited to, themed events, educational opportunities, social functions, spiritual opportunities, day trips, cultural outings, lectures, informational meetings, and volunteer opportunities. The Life Enrichment Leader shall collaborate with the Member Services Department to develop and continually update a file of resources available in the surrounding community to utilize and enhance the programming options available to members. The Life Enrichment Leader shall also assist in the recruitment and placement of volunteers to assist in the delivery of programming campus wide.

Maintenance Tech I

Maintenance Tech assists all maintenance personnel in the short term, preventative and emergency maintenance programs for the buildings, grounds and equipment at the campus. Responsibilities will include the completion of all assigned work orders and maintenance projects under the supervision of the Director of Facility Services.

MDS Coordinator

Minimum Data Set Coordinator is responsible for the development, maintenance and implementation of the standards of nursing practice and objectives as it relates to the Minimum Data Set requirements and with the established policies and procedures of the governing body in compliance with federal, state and local regulations. In additional to other key responsibilities, this position develops a proposed plan of care in conjunction with the interdisciplinary team ensuring that the plan of care is consistent with the current health assessment for every resident under the supervision of the Director of Health Care Administration.

Server provides food and beverage service tableside to residents, resident families and guests. This position maintains the cleanliness of the dining room, service stations, beverage machines, and all other work areas as well as the taking and filling of orders, servicing buffet lines, and accommodating residents’ mealtime requests under the supervision of the Dining Room Manager.

Valet Attendant

The valet attendant is responsible for welcoming residents and guest to the community, opening and closing car door(s), providing assistance in anyway necessary, and carefully parking vehicles in designated locations. This position is the first and last point of contact for our residents and guests allowing one opportunity for The Stratford to make a positive impression. Anticipating the unique needs of each resident/guest while maintaining excellent customer service, a positive attitude, and professional appearance at all times is a must.
